some time i have that bug:
i uploaded 1 file, but in folder i have 2 files
MOV_0060.mp4
MOV_0060 (1).mp4
it is same file, same size and i never upload MOV_0060 (1).mp4
upload procces was successful
HFS make that file for himself
some times ago other people upload to me 6 jpgs
but in folder was 10 jpg:
4 jpgs was doubled, 2 upload normally (8+2)

when the upload is attempted more times, HFS doesn't overwrite but gives another name.
It's an option, in Menu > upload
This is done to avoid a different user to overwrite good files with bad ones.
This is because HFS doesn't track who uploaded what, and cannot tell if you are entitled at overwriting. It should be better in the future.

the browser may have restarted the operation without warning you (because of failure)

HFS can track that "browsers failure" to log?
in my hfs log no error - only 1 upload operation (and doubles files)

sure.
i'm not sure logging "uploads" will do, but "requests" for sure.
